Insurance Reimbursement

Does FORGE WELLNESS CLUB bill insurance directly?

Yes. We are in-network with Premera for eligible therapeutic massage services. Insurance coverage is subject to your specific plan benefits, medical necessity, and any applicable referral or authorization requirements. A required intake consultation must be completed before your first appointment to verify your benefits and eligibility.

Does Forge Wellness Club accept HSA/FSA cards?

Yes! Forge Wellness Club accepts HSA (Health Savings Account) and FSA (Flexible Spending Account) cards for eligible services. Coverage and eligibility are determined by your individual HSA/FSA plan, and some services may require a Letter of Medical Necessity or physician referral for reimbursement. We recommend checking with your plan administrator if you're unsure about your benefits. We're also happy to provide an itemized receipt for your records.

Can I use my insurance benefits for services at FORGE WELLNESS CLUB?

If you have insurance other than Premera, you may be eligible for out-of-network reimbursement, depending on your plan. We recommend contacting your insurance provider before your appointment to verify your out-of-network massage therapy benefits and reimbursement eligibility. Upon request, we can provide a detailed receipt, a SUPERBILL, for you to submit to your insurance company.

What is a superbill?

A superbill is a detailed receipt that you can submit to your insurance company to request reimbursement for eligible out-of-network healthcare services. It includes the diagnosis and procedure codes your insurer needs to process your claim. Reimbursement is not guaranteed and depends on your specific insurance plan and out-of-network benefits.

Good to know: If you're using an HSA or FSA, you generally do not need a superbill just to use those funds. A standard receipt is usually enough. Superbills are primarily for submitting claims to insurance companies for potential reimbursement.

How do I submit a claim for reimbursement?

After receiving your superbill, you can submit it to your insurance company for possible reimbursement according to your plan's out-of-network claims process. Many insurance carriers allow you to submit claims online through their member portal, while others may require a paper claim form. Depending on your plan, you may also need to include additional documentation, such as a physician referral or Letter of Medical Necessity. Because reimbursement requirements, coverage, and processing timelines vary by insurance carrier and individual plan, we recommend contacting your insurer for specific instructions before submitting your claim.

Is reimbursement guaranteed?

No. Reimbursement is not guaranteed. Whether your insurance company reimburses you depends on your specific health plan, your out-of-network benefits, whether massage therapy is a covered service, and any medical necessity requirements. We recommend verifying your benefits with your insurance carrier before your appointment if you plan to seek reimbursement. Forge Wellness Club cannot guarantee coverage or reimbursement decisions made by your insurance company.
